2021-2022 Primary 1 Registration Notice

 

Please prepare for theoriginal and copy of the following documents：

(Please make copies in advance. Please understand the School does not providephotocopy service)

1.Birth Certificate:

Born in Hong Kong:	Birth Certificate of Hong Kong HKSAR Re-entry Permit or Hong Kong Passport(If the child lives in Mainland China)
Born inMainland China	Birth Certificate of China One-Way Permit  HKSAR Re-entry Permit

 1.Parent or Guardian’s Identification Document:

• Hong Kong Permanent Identity Card   or
• Resident Identity Card or other identity documents or other identity documents

 1.Address proof:

• Electricity bills, gas bills, water bills, telephone bills, maintenance bills  or
• Lease Contract  (Lease) or other supporting documents

   * The address in the document must be the same as the current address and that in the application form.

 1.One applicant’s recent photos：

• Such as an ID photo, kindergarten student photo, etc.   * It must be a photo taken in the last three months.

 

(A student status certification issued from 1 September 2020 to 31 August 2021)

• Transcript  or
• Student registration form (personal profile page) in the student handbook  or
• Certificate of merits of the kindergarten  or
• Letter of certification issued by the kindergarten.

 
* The above-mentioned documents must be stamped by the kindergarten.
